In most cases he actually does turn a profit for the studio. It's kind of like the guys who make those Meet the Spartans/Epic movies, if you're making a sequel to a big, successful blockbuster then you know that no matter the quality of the movie, it'll do good initial business purely on the strength of the original.
Uwe Boll basically applies this to videogames. He knows that if he makes a movie of say, Alone in the Dark then he's already got a built in audience of people who will see the movie before he's even shot a roll of film. It's simply a case of working out exactly how big an audience that is had then budgeting the movie accordingly. As long as he spends less on it than the fan-base suggests he's likely to make then it's easy money.
By doing this he's managed to carve out a cushie little career for himself and every so often he can go out and make something that's not based on a game.
